Who have passed or failed 10th standard
examination from any recognized board/ council are eligible for the post.
Candidate should be permanent resident of the same health sub-centre for which
she is applying.

HOW TO APPLY :
- Candidate will have to download the application from (on plain A4 Size paper) The duly filled in application should be enclosed with the following documents
- 2 copies of passport size recent coloured photo
- 1 self – addressed envelope of 23x10 cms size with postage stamp of Rs5/- Affixed thereon,Self attested photostal copies all certificate & mark Sheets in support of date of birth Educational qualification caste for sc/st/obc Self attested Photostat copies of valid residential proof Voter Card, Ration card
- Self attested Photostat copy of documents in support of marriage/ divorce/ widow. The envelope containing the duly filled in application should be suerscribed as APPLICATION FOR ENGAGEMENT AS ASHA NAME OF SUB-CENTER APPLIED FOR ……… AND NAME OF AREA……
The duly filled application along with all requisite
documents should be submitted by hand or sent by registered/ speed post with ad
so as to reach within 31.07.2018 to the office of respective block
development officer.
